Cefoxitin is a semisynthetic cephamycin beta-lactam antibiotic.
Cefoxitin is used for treatment of serious infections of the lower respiratory tract, skin and skin structure, bone and joint, and urinary tract; septicemia; gynecologic infections (including endometritis, pelvic cellulitis, and pelvic inflammatory disease); and intra-abdominal infections (including peritonitis and intra-abdominal abscess) caused by susceptible bacteria. Cefoxitin also is used for perioperative prophylaxis and has been used for treatment of uncomplicated gonorrhea+. Prior to and during cefoxitin therapy, the causative organism should be cultured and in vitro susceptibility tests conducted.
In serious infections, therapy may be initiated pending results of in vitro tests. Use of cefoxitin does not replace surgical procedures such as incision and drainage when indicated.

The following dosing information is available for CEFOXITIN SODIUM (cefoxitin sodium/dextrose, iso-osmotic):
When perioperative prophylaxis is indicated in patients undergoing surgery, administration of an appropriate anti-infective should be timed to ensure that bactericidal concentrations of the drug are established in serum and tissues by the time the initial surgical incision is made; therapeutic concentrations of the drug should then be maintained in serum and tissues for the duration of the procedure.
An IV dose of an appropriate anti-infective should be administered within 60 minutes before the initial incision. If cefoxitin is used prophylactically, the manufacturers recommend that the drug be given 30-60 minutes before the initial incision to ensure adequate cefoxitin tissue concentrations at the time of surgery.
A single preoperative dose usually is sufficient for perioperative prophylaxis. However, if surgery is prolonged (more than 3-4 hours), major blood loss occurs, or other factors are present that shorten the half-life of the drug (e.g., extensive burns), additional doses should be administered during the procedure to ensure adequate serum and tissue concentrations of the anti-infective throughout the procedure. For prolonged procedures in patients with normal renal function, some clinicians suggest that intraoperative doses be given during the procedure at intervals that correspond to 1-2 times the half-life of the drug (e.g., every 2 hours for cefoxitin, measured from the time the preoperative dose is initiated).
Intraoperative doses may not be warranted if there are factors that prolong the half-life of the drug (e.g., renal impairment).
For most procedures, the duration of prophylaxis should be less than 24 hours. There is no evidence to support continuing prophylaxis after wound closure or until all indwelling drains and intravascular catheters are removed.
Dosage of cefoxitin sodium is expressed in terms of cefoxitin.
The usual adult dosage of cefoxitin is 1-2 g every 6-8 hours, depending on the severity of the infection and the susceptibility of the causative organism. In severe, life-threatening infections, up to 12 g daily may be required.
The manufacturers suggest that adults with uncomplicated infections (e.g., pneumonia, urinary tract infections, cutaneous infections) receive 1 g IV every 6-8 hours, those with moderately severe or severe infections receive 1 g IV every 4 hours or 2 g IV every 6-8 hours, and those with infections requiring higher dosage receive 2 g IV every 4 hours or 3 g IV every 6 hours.
The duration of cefoxitin therapy depends on the type of infection. In infections caused by group A beta-hemolytic streptococci, therapy should be continued for at least 10 days.
The usual dosage of cefoxitin for pediatric patients 3 months of age or older is 80-160 mg/kg daily given in 4-6 equally divided doses. The maximum dosage is 12 g daily. Safe use of cefoxitin in infants younger than 3 months of age has not been established.
The American Academy of Pediatrics (AAP) recommends that pediatric patients beyond the neonatal period receive cefoxitin in a dosage of 80 mg/kg daily given in 3-4 divided doses for treatment of mild to moderate infections or 160 mg/kg daily given in 4 divided doses for treatment of severe infections.
The duration of cefoxitin therapy depends on the type of infection. In infections caused by group A beta-hemolytic streptococci, therapy should be continued for at least 10 days.
In patients with impaired renal function, doses and/or frequency of administration must be modified in response to the degree of impairment, severity of the infection, and susceptibility of the causative organism. Dosage adjustments are not necessary in adults with creatinine clearances greater than 50 mL/minute.
In adults with creatinine clearances of 50 mL/minute or lower, the manufacturers recommend a loading dose of 1-2 g followed by a maintenance dosage based on the patient's creatinine clearance. (See Table 1.)

The usual adult dosage of cefoxitin is 1-2 g every 6-8 hours, depending on the severity of the infection and the susceptibility of the causative organism. In severe, life-threatening infections, up to 12 g daily may be required.
The manufacturers suggest that adults with uncomplicated infections (e.g., pneumonia, urinary tract infections, cutaneous infections) receive 1 g IV every 6-8 hours, those with moderately severe or severe infections receive 1 g IV every 4 hours or 2 g IV every 6-8 hours, and those with infections requiring higher dosage receive 2 g IV every 4 hours or 3 g IV every 6 hours.
The duration of cefoxitin therapy depends on the type of infection. In infections caused by group A beta-hemolytic streptococci, therapy should be continued for at least 10 days.
The usual dosage of cefoxitin for pediatric patients 3 months of age or older is 80-160 mg/kg daily given in 4-6 equally divided doses. The maximum dosage is 12 g daily. Safe use of cefoxitin in infants younger than 3 months of age has not been established.
The American Academy of Pediatrics (AAP) recommends that pediatric patients beyond the neonatal period receive cefoxitin in a dosage of 80 mg/kg daily given in 3-4 divided doses for treatment of mild to moderate infections or 160 mg/kg daily given in 4 divided doses for treatment of severe infections.
The duration of cefoxitin therapy depends on the type of infection. In infections caused by group A beta-hemolytic streptococci, therapy should be continued for at least 10 days.
In patients with impaired renal function, doses and/or frequency of administration must be modified in response to the degree of impairment, severity of the infection, and susceptibility of the causative organism. Dosage adjustments are not necessary in adults with creatinine clearances greater than 50 mL/minute.
In adults with creatinine clearances of 50 mL/minute or lower, the manufacturers recommend a loading dose of 1-2 g followed by a maintenance dosage based on the patient's creatinine clearance. (See Table 1.)

In adults with renal impairment undergoing hemodialysis, a loading dose of 1-2 g should be given after each dialysis period followed by a maintenance dosage based on the patient's creatinine clearance. (See Table 1.)
If cefoxitin is used in pediatric patients with renal impairment, dosage adjustments should be made similar to those recommended for adults.

Patients may be at increased risk of infections or bleeding during this time. Monitor complete blood counts within one month of therapy. If results indicate leukopenia or thrombocytopenia, dosimetry should be used to determine a safe sodium iodide I 131 activity.(1) DISCUSSION: Hematologic disorders including death have been reported with sodium iodide I 131. Selected Cephalosporins & Penicillins/Probenecid SEVERITY LEVEL: 3-Moderate Interaction: Assess the risk to the patient and take action as needed. MECHANISM OF ACTION: Probenecid impairs the clearance of some cephalosporins and penicillins via inhibition of renal anion transporters in the proximal tubule.(49) It has also been hypothesized that probenecid may affect tissue distribution of cephalosporins.(1-5) CLINICAL EFFECTS: The concurrent administration of probenecid may result in increased maximum concentration (Cmax), area-under-curve (AUC), and half-life of the cephalosporin or penicillin.(49) While this may improve antibiotic efficacy,(46-48) increased levels may also increase the risk for antibiotic-associated nephrotoxicity.(4) PREDISPOSING FACTORS: Underlying renal dysfunction may increase the risk for nephrotoxicity. PATIENT MANAGEMENT: In patients receiving the combination to improve antibiotic efficacy, monitor for antibiotic adverse effects and consider monitoring renal function. In patients receiving probenecid therapy to prevent or treat hyperuricemia, exposure to the antibiotic will be increased. A decrease in antibiotic dose or frequency may be required. The US manufacturer of piperacillin-tazobactam states probenecid should not be coadministered with piperacillin-tazobactam unless the benefit outweighs the risk.(50) DISCUSSION: Concurrent use of probenecid with a cephalosporin or penicillin may cause an increase in the Cmax, AUC, and an increased elimination half life of the antibiotic.(6-8,49) This may be beneficial or necessary in difficult to treat infections,(46-48) but an increased risk for adverse effects should be expected. Antibiotics not dose adjusted for concurrent use with probenecid may be associated with an increased risk for adverse effects, such as nephrotoxicity. Probenecid administered concurrently with piperacillin-tazobactam prolongs the half-life of piperacillin by 21% and tazobactam by 71%. In a study in 8 healthy males, concurrent administration of probenecid (1 g) with piperacillin (1 g IM) increased piperacillin's Cmax and AUC by 30% and 60%. Renal clearance was reduced by 40%.(51) The cephalosporins affected by probenecid include cefazolin,(9-11) cephacetrile,(12,13) cephaloglycin,(14,15) cephalexin,(16-21) cephradine, (22-23) cefoxitin,(24-28) cefadroxil(29), cefaclor,(23) cefamandole,(30) ceftizoxime,(31,32) cefuroxime,(33,34) cefprozil,(35) cefonicid,(36) cefmetazole,(37) cefmenoxime,(38) and cefditoren.(39) Probenecid has been shown not to affect moxalactam,(4,40,41) ceforanide, (4,42), cefoperazone, ceftazidime(4,34,43) or ceftriaxone.(4) |

Slt Anticoagulants (Vit K antagonists)/Slt Cephalosporins SEVERITY LEVEL: 3-Moderate Interaction: Assess the risk to the patient and take action as needed. MECHANISM OF ACTION: The exact mechanism is unknown, but may involve a combination of cephalosporin induced platelet inhibition and alteration of gut flora. CLINICAL EFFECTS: Concurrent use of some cephalosporins may increase the hypoprothrombinemic effect of the anticoagulant with possible bleeding. PREDISPOSING FACTORS: High doses, hepatic and/or renal impairment, and poor nutrition may increase the risk of bleeding. The risk for bleeding episodes may be greater in patients with disease-associated factors (e.g. thrombocytopenia). Drug associated risk factors include concurrent use of multiple drugs which inhibit anticoagulant/antiplatelet metabolism and/or have an inherent risk for bleeding (e.g. NSAIDs). PATIENT MANAGEMENT: Monitor prothrombin activity and adjust the anticoagulant dosage accordingly. Consider using an alternative antibiotic. If concurrent therapy is warranted, monitor patients receiving concurrent therapy for signs of blood loss, including decreased hemoglobin, hematocrit, fecal occult blood, and/or decreased blood pressure and promptly evaluate patients with any symptoms. When applicable, perform agent-specific laboratory test (e.g. INR, aPTT) to monitor efficacy and safety of anticoagulation. Discontinue anticoagulation in patients with active pathologic bleeding. Instruct patients to report any signs and symptoms of bleeding, such as unusual bleeding from the gums or nose; unusual bruising; red or black, tarry stools; red, pink or dark brown urine; acute abdominal or joint pain and/or swelling. The time of highest risk for a coumarin-type drug interaction is when the precipitant drug is initiated or discontinued. Contact the prescriber before initiating, altering the dose or discontinuing either drug. DISCUSSION: Although the majority of cephalosporins that have been documented to interact with anticoagulants have a NMTT side chain, there are reports of interactions with cefazolin, cefoxitin, ceftaroline, and ceftriaxone as well. The time of highest risk for a coumarin-type drug interaction is when the precipitant drug is initiated or discontinued. A large systematic review was performed on 72 warfarin drug-drug interactions studies that reported on bleeding, thromboembolic events, or death. Most studies were retrospective cohorts. A meta-analysis of 11 of those studies found a higher rate of clinically significant bleeding in patients on warfarin and antimicrobials (OR=1.63; 95% CI 1.45-1.83). Increased bleeding risk was also seen in subgroup analyses with cephalosporins (OR=1.50; 95% CI 1.21-1.86). |

Reproduction and teratologic studies in mice and rats using parenteral cefoxitin doses 1-7.5 times the maximum recommended human dose have not revealed evidence of impaired fertility or harm to the fetus, although a slight decrease in fetal weight was observed. When used in rabbits, cefoxitin was associated with a high incidence of abortion and maternal death; however, this was considered to be an expected consequence of the rabbit's unusual sensitivity to antibiotic-induced changes in intestinal flora rather than a teratogenic effect.
There are no adequate and controlled studies to date using cefoxitin in pregnant women. Because animal reproduction studies are not always predictive of human response, cefoxitin should be used during pregnancy only when clearly needed.
