Acyclovir
Renal-Dose interval adjustment necessary for CrCL<25 mL/min. for high dose (800 mg oral) treatment. For IV therapy: CrCL >25-50 mL/min, adjust interval to 12 hours; CrCL>10-25mL/min, adjust interval to 24 hours; CrCL<11mL/min, decrease dose by half and use a 24 hour interval. Renal toxicity more common in the elderly. Neuro/Psych-Dizziness, somnolence, hallucinations, confusion and coma are reported more commonly in the elderly.
